To be able to process your sample efficiently it is important for us to a clear instruction outlining the patient details and tests that you would like us to process for you.

It is essential for us to have a Request form completed by a Doctor for each patient.

You will need to complete:

• Your details and to where you would like the Results sent
• Patient Details - please provide full name, date of birth, gender
• Tests required
• Invoicing instruction - do we invoice you or your patient?

Please complete the Request form and make sure it is enclosed with the samples taken at your practice. Label the samples carefully with your patient's full name, and date of birth.  Please ensure that samples posing a particular risk of infection  are double bagged and the request form is labelled 'HIGH RISK'.
