Sample Identification & Labelling |

TDL needs to demonstrate continued compliance with the standards set by both Clinical Pathology Accreditation (CPA) and good clinical practice for sample and form labelling. We need one request form for each patient with a minimum of three unique identifiers on both the sample and the form.

With effect from the 1st October 2007 the updated policy in Sample Reception gives instruction for all inadequately labelled samples, of any type, to be returned to the referring practice for corrective action before they can be processed by the laboratory.
If samples need to be returned to you, this will inevitably cause a delay with their analysis.
